Electronic Medical Record Submission
Please enter your QIO ID, Electronic Medical Record Key, and
select your Electronic Medical Record file to submit.
- The QIO ID is 13 characters including dashes. Letters are UPPERCASE.
- The EMR Key is six UPPERCASE characters.
- The file must end in .pdf with a lower-case file name.
- Verify all uploaded files are searchable.
- DO NOT upload password protected files. Doing so will delay case processing.
- DO NOT upload encrypted files. Doing so will delay case processing.
- The maximum file size is 1 GB. If your file is larger than 1 GB, we recommend splitting it into 10,000 page increments.
- Please submit a single complete medical record with the medical record request attached as the first page.
